Sleep Apnea Therapy Insights & Truths
About CPAP - PAP or positive airway pressure, is the most often prescribed form of therapy for those suffering with Obstructive Sleep Apnea (OSA) because it is the most effective. Truth is, if you can tolerate the mask it works. The challenge is that 50% of those prescribed PAP discontinue its use by the first year and by the fifth year 83% have discontinued its use, leaving only 17% effectively in therapy ...
About Inspire - it's probably one of the most attractive options available with certainly the best marketing phrase - "No Mask. No Hose. Just Sleep". It's also silently discreet and patients are more than likely to adhere to it due to its simplicity and convenience. Disadvantage is that, although it is generally an outpatient procedure, it is still surgery that involves an implanted device. However, the surgery does bring with it some predictability because a prerequisite is a procedure called DISE, or drug induced sleep endoscopy, which identifies both the level and the direction of the collapsing airway ...
About GLP Medications - are a pharmaceutical approach to addressing Sleep Apnea, not by treating the airway blockage itself but by helping patients lose weight which in turn reduces or eliminates their OSA. Because obesity is a major modifiable risk factor for sleep apnea, these medications can play a crucial role in improving patients' overall health and quality of life. Although this category of medication brings with it some side effects, risks and contraindications, they are not listed here, since this body of knowledge is new and continually evolving - point being - for patients considering this approach check the literature and research for current information ...
About Mandibular Advancement Devices, more commonly called Oral Appliance Therapy (OAT) - although OAT finds itself last on this list, it's for no other reason than because it's what Dr. John L. Bouzis provides, it should be at least number two because it involves no surgery and/or ingestion of prolonged medication. Oral Appliances are much easier to wear than CPAP with a near 90-95% compliance rate i.e. patients can easily wear them as they are custom fit, comfortable and easier to travel with than CPAP. However, no therapy is without side effects and although that also rings true for OAT, they are minimal and non-systemic in nature like medications or surgery. Still to be transparent, although it is largely preventable by using what is termed an AM Aligner each morning after removing the appliance, some patients can experience minor tooth movement while others may experience some transient jaw discomfort. With respect to jaw discomfort and for those that have been struggling with headaches, because the appliances are not only custom fit at delivery but also custom fabricated, they can be designed to simultaneously treat the Sleep Disorder, TMD-TMJ and Musculoskeletal problems as well as Migraine Headaches whereas no other form of therapy possesses that capability ...
